Ideal candidates will possess:
Bachelor’s Degree (Required)
3+ years of experience in case management/service coordination
Existing connections with appropriate community referrals, medical providers, and other professionals
Service Coordinator Qualifications & Skills:
Excellent communication skills, both verbal and written
Listening actively to understand and empathize with participants and convey information clearly and effectively to foster trust
Empathy and compassion are vital qualities for service coordinators especially when working closely with participants with health challenges and navigating complex care systems
Strong organizational and time management skills are necessary to manage multiple participant appointments and tasks efficiently
Cultural competence and sensitivity are crucial for effectively working with participants from diverse backgrounds and communities
The ability to collaborate effectively, communicate with team members, and work cohesively towards common goals is essential for providing holistic care and support to participants
