Ideal candidates will possess:

• Bachelor’s Degree (Required)

• 3+ years of experience in case management/service coordination 

• Existing connections with appropriate community referrals, medical providers, and other professionals

Service Coordinator Qualifications & Skills:

• Excellent communication skills, both verbal and written

• Listening actively to understand and empathize with participants and convey information clearly and effectively to foster trust

• Empathy and compassion are vital qualities for service coordinators especially when working closely with participants with health challenges and navigating complex care systems

• Strong organizational and time management skills are necessary to manage multiple participant appointments and tasks efficiently

• Cultural competence and sensitivity are crucial for effectively working with participants from diverse backgrounds and communities

• The ability to collaborate effectively, communicate with team members, and work cohesively towards common goals is essential for providing holistic care and support to participants
