Managed Long-Term Care (MLTC):

MLTC programs typically offer a wide range of services, including home health care, personal care, skilled nursing, therapy services, medical equipment and supplies, and other support services tailored to the individual's needs.
Nursing Home Transition and Diversion (NHTD) Waiver:
NHTD waivers specifically focus on helping individuals transition back to community-based settings or diverting them from entering nursing homes altogether. The emphasis is on providing support services and resources to enable individuals to live independently in their homes or community settings.
The waiver program provides various services to support individuals in transitioning from nursing homes. These may include home and community-based services such as personal care assistance, home modifications, assistive technology, respite care, transportation, and other supportive services aimed at maintaining individuals' independence in community settings.
While both MLTC and NHTD Waiver programs aim to provide long-term care services to individuals, they differ in their focus, target population, services provided, eligibility criteria, and administration.
